Safety at Feltham prison has reached a crisis point, new figures suggest. It comes as a workers' union has warned the increase of violence in prisons across England is "totally out of control".

It has the highest levels of violence of any prison in England and Wales, an inspection finds. The HM Inspectorate of Prisons calls for a "change of culture" at HMP Feltham B in west London.
