Fatal crashes of commercial aircraft in the U.S. have become a rarity. The last was in 2009 near Buffalo, New York.

The National Transportation Safety Board determined Alaska had failed to properly maintain the plane’s jackscrew, part of a system that helps control the aircraft’s angle of flight.
